Collecting and using your data

When you book a treatment with me, I will need to record your name and address in order to attend to your treatment. I will also need to collect your name, address, telephone number, email address, and date of birth so that I can contact you to discuss your treatment or arrange future appointments. Your date of birth is required for identification purposes. I will also ask you to sign a consent form allowing me to provide the treatment and confirming that you have disclosed any medical contraindications you may have.

By booking and receiving a treatment with me, you agree to allow me to store your personal information for the purpose of carrying out the treatment, contacting you regarding your current and future appointments, and booking further treatments. I will not use your personal information for marketing or any other purposes without your consent.
